To Request Aid

Click on the "Request Aid" button below. PLEASE NOTE:  you will need to send us four items: (1) this completed Aid Application form, (2) a Care Credit Application with their approval or denial (we guide you to this from our Aid Application form), (3) an estimate of how much your pet's care will cost, and (4) a photo of the pet in need of assistance. ALL FOUR of these must be completed for care. We realize you may be facing an urgent situation, so we will do our best to help you with this process.  The fastest way we can respond if to email us these four items to [email protected]

Guidelines for funding:

Pets receiving donations must have the potential for both quantity and quality of life -- and pets must live in Maine

The maximum percentage of the bill covered is not to exceed 50% of the medical care ill with a cap of $500.

NOTE:  You must fill out a Care Credit application for financial screening prior to our review - if you do not quality for their assistance, we then consider how we can help

Awarded aid must be used within 90 days

FVH staff, MPF Board members may recommend cases for aid but are not eligible for funding themselves

Compassionate need and charitable organizations in Maine may apply to the board with a proposal and be considered on a case-by-case basis.

Funds may not be used for cremation services or products

Funds are directed to the care / cause not to an individual.
